Puzzling Fuel Delivery Problem

I'm having a fueling problem now.

Symptoms:

When the car is cold, such as a morning start, the car has no problem meeting requested fuel pressure. The car drives fine and shows no trouble codes. However, as the car starts to get warm my fuel rail pressure differences start to increase dramatically.

When the car has warmed up, the differences in rail pressure can range all the way from 30 bar at idle, to 60+ bar under load. The car will go into limp mode, and I get the code 008851 (N276 mechanical malfunction - lower limit exceeded) and 000135 (fuel rail/system pressure: too low).

The cam and follower are fine.

Is this sounding like a Fuel Pressure Regulator problem? Possibly a Fuel Relief Valve problem? It's interesting that this problem only surfaces after the motor starts to warm up.
