Rev D failure?

So I finally got a chance to install the GFB DV+ kit and when I pulled out my Rev D here is what it looked like. It's only been installed for 6-7K miles. I figured this might be the cause of my boost gauge going up and down like crazy during WOT. I swapped the internals with the GFB kit and the car feels MUCH better.. a lot more peppy than before, however, I'm still having issues with my boost going up and down during WOT so maybe that's an entirely different issue(n75 valve maybe?). I can run some scans with vagcom but I'm not sure what I need to log.

1. How could this of happened and has anyone seen this before?
2. Where is the little piece of plastic that broke off?
3. Is it ok that I used the GFB kit with this DV?

I don't see how that's possible and I was pretty gentle when removing it.
