An offset of 45 is still pretty good and spacers would hurt cause they would bring the offset down. If you for example have wheels with an offset of 45 and you use for example 10 mm spacers the offset of those wheels would go to 35 and poke out.

I'm looking at RS4 wheels that are 19X8, the only problem is the greatest offset i have found is 45

I have 17x8, ET 45 and they are pefectly fine, they look beefy and stick out a very small bit more than stock, but they in no way rub.
